Clinical Measurement and Significance of Leg Length & Iliac Crest Height Discrepancies

Abstract: This literature review article examines the clinical relevance of various amounts of leg length discrepancies (LLD) in humans, followed by a review of the validity and reliability of various LLD measurement methods. Depending on the activity level and the type of effects looked at, LLDs of > 5 mm and > 10 mm negatively impact the spine and lower extremities. The x-ray measurements of relative femoral head or iliac crest height are valid measures of LLD, while the reliability of palpating and visually estimating relative iliac crest height is question- able. In absence of imaging techniques, the development of a more reliable clinical method of LLD measurement is warranted.

The PalpationMeter (PALM)

Abstract: The PalpationMeter (PALM) is a caliper-inclinometer combination instrument with palpation tips that lock to the fingers of an examiner while he or she palpates landmarks during the evaluation of bony structure or joint mobility. The instrument, the advantages of palpation controlled instrument - body interfacing, PALM measurements of pelvic tilt angles and leg length discrepancies, the need for anthropometric data on pelvic structure and the need for a validating x-ray comparison study are described.

Abstract: Patellofemoral (PF) instability or recurrent PF subluxation is a variant of PF pain syndrome, in which there is actual subluxation of the patella out of the femoral trochlea. Knowledge about and management of this condition has advanced quickly over the past decade. This paper reviews the recent literature in this area.

A Qualitative Appraisal of Professional Medical Attitudes to Orthopaedic Manipulative Therapy y

Abstract: The purpose of this paper was to explore the validity of a perception, that the treatment of neuro-muscular-articular problems (N.M.A) by manipulative therapists is not widely known or understood by medical practitioners in South Africa. If this perception is validated, it could explain a cause of the suspected lack of effective referral of patients who could potentially benefit from such therapy. A questionnaire format was the method of investigation and the universe was 467 medical practitioners, drawn from the greater Johannesburg area. One hundred and forty respondents formed the sample base. Results indicate that there are areas of deficit in medical practitioner's knowledge and belief in the principles and methods of orthopaedic manipulative therapy (O.M.T). The traditional method of communication in the form of publications in physiotherapy journals has failed to raise the desired level of awareness. Orthopaedic Manipulative Therapist's (O.M.T's) need to be proactive in developing strategies if they are to create, among medical practitioners, an increasing awareness of the scope and potential benefit of O.M.T.
